Missing baserunner

Hidalgo walked to set up a bases loaded situation for the Astros in the bottom of the 9th. Next batter up is Berkman. He hits a ground ball and a play is made at the plate. Biggio out, Bagwell to third, Hidalgo to second, Berkman on at 1st. But as you can see below, Bagwell still shows at 2nd and Hidalgo is gone completely.

One out is recorded for the Astros. Narrative says bases are still loaded.
Next batter, Rader, grounds out and the runners move up a base and Voila! Hidalgo is back. However there are now two outs showing.

The next batter, Rader, grounds out and the runners move up to 2nd and 3rd and Voila! Hidalgo pops back up at 1st base with 2 outs showing.

Weird game ended on a bases loaded hit by pitch. Note that Tug McGraw did not load the bases but is charged with the loss, while Carlos Lee pinch hit for Billy Wagner and was lifted for a pinchrunner, yet gets credited with the win.
