Substitution bug?

Bottom of the 15th, runner at first two out pitcher due up. Go to the bench for Dale Mitchell to pinch hit. The change shows on the screen and the pitch and hit buttons are pushed. Result comes up as a 36-33 for THE PITCHER. Result is a pop up for out number three. Top of the 16th is announced along with the result of a 31-9 for the pinch hitter. That 9 would have been a hit with the runner on first against the A reliever. Instead Mitchell is out of the game and a new pitcher is forced to come on for the 16th. No out recorded for the top of the inning even though the narrative shows one.
